Support Coordination
▹Looking for someone who really gets you?
At SWIS, we’re not here to take over. We’re here to work alongside you. We help you understand your plan, figure out what actually matters to you, and connect you with supports that fit your life — not just what’s available.
Over time, we support you to build confidence and take more control of your supports and decisions, with the right team behind you.
We’re big on keeping things real and keeping it about you. Your goals, your choices, your life. We take the time to get to know you properly — what a good day looks like for you, what’s important, and what you want to work towards.
▹Supporting You…and Your People
We also know that friends and family matters. We work alongside the people around you, making sure everyone is on the same page and working together to support you, because good supports don’t just work on paper — they work in real life.
At the end of the day, it’s about helping you live your best life, your way — with the right support around you to make it happen.
We can make NDIS easier for you!
▹Finding the Right Fit
Not every support is the right support — and that matters.
We take the time to get to know you properly so we can connect you with people who actually fit your life. Not just who’s available, but who understands you, your goals, and how you like things done.
We don’t lock you into anything. You always have choice and control, and we’ll keep working with you until it feels right.
Because the right fit makes all the difference.
▹Making the Most of Your Plan
We help you understand your plan and use it in a way that actually works for your life — not just on paper.
From setting up the right supports to making sure everything is running smoothly, we keep things on track and make sure your plan is working the way it should.
If something’s not right, we step in and sort it. Because your plan should be helping you move forward, not holding you back.
▹Less Stress, More Support
The NDIS can feel like a lot — that’s where we come in.
We take care of the behind-the-scenes stuff so you don’t have to. From organising supports to dealing with providers and sorting out issues when they pop up, we keep things moving and take the pressure off you and your family.
So you can spend less time stressing about the NDIS, and more time focusing on what matters to you.
▹With You Every Step
We’re with you from the start and we stay alongside you.
As things change, we check in, support you through it, and help keep everything on track. If something’s not quite right or you need a hand, we’re there to help sort it out.
You’re not doing this on your own — you’ve got people who know you and are there when you need it.

The NDIS (National Disability Insurance Scheme) is a government program that provides funding to people with disability to help them live more independently and reach their goals.
Your plan is built around you — what you need, what you want to work towards, and what supports will help you get there. This can include things like support workers, therapy, equipment, and help to get out into the community.
The NDIS can feel confusing at first, and that’s pretty normal. There’s a lot to take in and it doesn’t always make sense straight away.
That’s where we come in — to help you understand it, use it properly, and make sure it’s actually working for you.
Find out more about the NDIS via these links or contact us today and we’ll help you understand what this means for you.

Support Coordination is about helping you make sense of your NDIS plan and actually use it in a way that works for you. We help you understand your funding, connect you with the right supports, and get everything set up so it runs smoothly. If things change or something’s not working, we step in and help sort it out.
There are different levels of Support Coordination, depending on what you need:
Level 2 – Support Coordination:
Helps you understand your plan, connect with providers, and keep your supports running the way they should.Level 3 – Specialist Support Coordination:
For more complex situations. This involves a higher level of support to manage challenges, reduce risk, and bring multiple services together when things aren’t straightforward.
At the end of the day, it’s about making sure your supports are working for you — and helping you build the confidence to take more control over time.

Support Coordination helps take the pressure off and makes the NDIS easier to manage.
We help you understand your plan, connect you with the right supports, and make sure everything is set up and working the way it should. If something’s not right or needs changing, we step in and help sort it out.
It also means you’ve got someone there who knows what’s going on, can answer your questions, and helps you make the most of your plan, without you having to figure it all out on your own.

It comes down to finding someone who feels right for you!
A good Support Coordinator should listen, understand what matters to you, and work alongside you — not take over. They should be easy to talk to, responsive, and actually follow things up.
It’s also important they know the NDIS well, have good local connections, and can help you navigate challenges when things don’t go to plan.
At the end of the day, you should feel comfortable, supported, and in control. If it doesn’t feel right, you can always change! It’s your choice
-
That’s completely fine! It’s your choice who you work with. If you’re happy with your current Support Coordinator, there’s no need to change. But if things aren’t working, you’re not getting the support you need, or you just want something different, you can switch at any time.
We can help make that process easy and make sure there’s a smooth handover so nothing gets missed.
At the end of the day, you should feel supported and comfortable with who you’ve got…and if you don’t, you’ve got options.
