South West Individual Support (SWIS) provides practical, reliable disability support across South West Victoria. We work alongside our participants to make sure supports are the right fit, easy to understand, and responsive when things change.

▹Why SWIS was created

SWIS was created because too many people in regional communities were struggling to access the right support. Services were often limited, inconsistent, or not built around the person. We wanted to create something better — support that is flexible, responsive, and centred around what matters to each participant

We focus on supports that are practical, consistent, and built around real life. That means taking the time to understand each person properly, matching supports carefully, staying involved when things are complex, and adjusting when something is not working.

Our team brings experience across disability, nursing, mental health, aged care, community services, early intervention, and the NDIS. The team includes people with backgrounds as nurses, Local Area Coordinators, NDIA staff, and community service professionals, giving SWIS a strong understanding of both direct support and how systems work.

We work with participants who want support that is reliable, practical, and built around their goals. We are especially well suited to people who need Support Coordination, people with complex or changing circumstances, and participants living in regional communities where the right support can be harder to find.

Meet Our Team

  • Director

    Fiona has over 30 years of experience across disability, mental health, and nursing. She is a qualified nurse and has worked in both clinical and community settings, supporting people with a wide range of needs.

    Through her experience, Fiona saw the challenges people in regional communities face when trying to access the right support. She founded SWIS to create a service that is more responsive, practical, and built around the individual.

    Fiona is committed to ensuring our participants receive support that is consistent, respectful, and genuinely works in their day-to-day lives.

  • Senior Support Coordinator

    Alesha has 18 years of experience in disability and community services, including approximately 7 years working within the NDIA. She brings a strong understanding of how the NDIS works and how to navigate it effectively.

    Alesha also brings lived experience with disability, which shapes the way she supports participants and families. She combines this with a practical, straightforward approach, focusing on real outcomes.

    She works closely with participants and our staff to ensure their supports are clear, achievable, and working properly, particularly in more complex situations.

  • Support Coordinator/Administration

    Shanae is a Registered Nurse and Registered Midwife with experience across disability and community settings. She brings a strong clinical understanding and a calm, organised approach to her role.

    She supports both participants and the day-to-day operations of SWIS, ensuring supports are structured, communicated clearly, and delivered safely. Shanae focuses on making sure participants feel supported, informed, and confident in their supports.

  • Support Coordinator

    Amy has experience in early childhood education and disability support. She brings a warm and supportive approach and enjoys working closely with participants and families.

    She focuses on building strong relationships and ensuring supports are tailored, consistent, and meaningful. Amy works alongside participants to help them feel confident and supported in their everyday lives.

  • Support Coordinator

    Bernie is a Registered Nurse and has 20 years experience across aged care, community, and primary health settings, supporting people with a wide range of needs. She brings a practical and steady approach to her work.

    Bernie focuses on making sure participants understand their supports and feel confident in how they are delivered. Bernadette values consistency, reliability, and building positive working relationships.

    Darci has spent over 20 years working across community services, including youth work, housing, community development, and the disability sector. She works closely with participants to ensure supports are not only in place, but working effectively.

    Darci focuses on clear communication, follow-through, and making sure nothing is overlooked.

    Christine has extensive experience across disability, aged care, and community services. She brings a broad understanding of how different systems connect and how to support people through change.

    Christine also brings lived experience with disability, which informs her approach and understanding of participant needs. She takes a calm and thoughtful approach and focuses on building stable, consistent supports.

    Kayla has over 13 years experience working in disability support and participant-focused care, as well as lived experience.

    She is approachable, supportive, and focused on building strong connections with participants.

    Kayla also brings lived experience with disability, which gives her a genuine understanding of the importance of supports that are respectful, consistent, and tailored to the individual.

    She works alongside participants to ensure their supports are practical and suited to their needs.

  • Support Coordinator

    Josh has strong experience across the NDIS and disability sector, Prior to this, he spent six years in the Defence Force, where he developed the discipline, reliability, and problem-solving skills he now brings into his role.

    He has a solid understanding of how the NDIS works and, more importantly, how to make it work in a practical way for the people he supports.

  • Support Coordinator

    Stef has several years of experience supporting NDIS participants, beginning her work in the sector as an independent support worker before transitioning into Support Coordination in 2024.

    Prior to this, Stef spent over 20 years in medical administration, primarily within General Practice. This has given her a strong foundation in communication, organisation, and navigating health-related systems, which she brings into her current role.

    Stef is known for her steady and methodical approach. She focuses on helping participants make sense of their options, keep things on track, and ensure supports are practical and working as intended.

    She values being a consistent and reliable presence, supporting participants as they work towards their goals and build confidence over time.

  • Support Worker

    Alan has four years of experience working as a Disability Support Worker, supporting participants both at home and out in the community.

    He takes a relaxed and approachable approach to his work, focusing on helping participants stay engaged in the things they enjoy and maintain their independence. Alan works alongside our participants in a way that feels natural and supportive, adapting to what works best for each individual.

    Outside of work, Alan enjoys spending time outdoors, particularly fishing, camping, and four-wheel driving. He also enjoys footy and staying active.

  • Support Worker

    Fraser has been working as a Disability Support Worker for the past five years, supporting people in their homes and in the community.

    Fraser is passionate about supporting people to have genuine choice and control, and works alongside our participants in a way that promotes independence and aligns with what matters to them.

    He is known for his steady, friendly nature and his ability to help participants feel at ease in their supports.

    Outside of work, Fraser enjoys staying active, spending time outdoors, and keeping busy with hobbies such as camping and gardening.

    Holly has 16 years of experience working as an Enrolled Nurse, supporting people across a range of healthcare settings. She joined as a Support Worker and brings a strong clinical background along with a calm and practical approach.

    Having grown up in the local area, Holly values being part of the community and building genuine connections with the people she supports. She takes the time to understand each participant and works alongside them in a way that feels comfortable, respectful, and suited to their needs.

    Holly enjoys supporting participants with their day-to-day routines and finds it rewarding to make a positive difference, even in the small things, helping people feel supported and valued.

  • Support Worker

    Kiarne has over five years of experience in support roles, working with people of all ages across a range of needs.

    She brings a positive, down-to-earth approach and enjoys getting to know each participant, supporting them to build confidence and independence in a way that suits them. Kiarne has a strong interest in community access and enjoys supporting participants to get out, try new things, and stay connected.

    Outside of work, she enjoys playing netball and spending time at the racetrack.

  • Support Worker

    Larissa is originally from the Mornington Peninsula and has been part of the Colac community for almost seven years. She brings over 25 years of experience working in customer-focused roles across hospitality, retail, and community environments.

    Her background includes long-term roles in local businesses and schools, where she developed strong communication skills, reliability, and the ability to connect with people from all walks of life.

    Larissa made the transition into disability support to pursue a more meaningful and rewarding role. She enjoys supporting participants to get out into the community, take part in programs, and work towards their goals in a way that feels safe and comfortable. She particularly enjoys being involved in group programs and school holiday activities, helping create a positive and engaging environment.

    Lucinda has over 35 years of experience supporting people with disabilities across all stages of life, from early childhood through to older adults.

    She has extensive experience working with complex communication needs and behaviour support, with a background in autism, trauma-informed practice, and dual disability.

    Lucinda focuses on building strong relationships and supporting participants to develop confidence, skills, and independence, with a strong emphasis on capacity building and person-centred practice.

  • Support Worker

    Lueanne is local to the Colac area and brings experience across hospitality and aged care, including four years working as a Food Services Assistant. She has also completed commercial cookery.

    She takes a warm and practical approach and enjoys supporting participants both at home and out in the community, particularly through cooking-based activities.

    As a mum of three, including a neurodiverse child, Lueanne brings lived experience and a genuine understanding of the day-to-day realities of support.

    Outside of work, she enjoys country life and lives on a dairy farm with her family.

  • Support Worker

    Raz has a strong, hands-on background and brings a practical approach to her role. She is a qualified pastry chef and holds a Certificate IV in Childcare, with experience supporting foster children.

    Before joining SWIS in 2023, Raz spent eight years at Colac Area Health in a supervisory role, developing strong skills in working with people and managing day-to-day supports.

    Raz enjoys getting to know participants and supporting them in a way that feels natural and engaging. She is often involved in cooking-based supports, helping create a relaxed and social environment where participants can build skills and confidence.

    Outside of work, Raz enjoys spending time with her partner and children and staying active with family life.
